WDSC TERMS & CONDITIONS

During the registration process, you will be asked to agree to the following Terms & Conditions:

1. Medical Release Waiver
I certify that I am the parent or legal guardian for my child(ren). I hereby give my permission for any
supervisor, coach or other team administrator associated with the Winskill Dolphins Swim Club to
seek and give appropriate medical attention for our child(ren) in the event of accident, injury, illness.
I will be responsible for any and all costs associated with any necessary medical attention and/or
treatment.

I hereby waive, release and forever discharge Winskill Dolphins Swim Club and associated
supervisor, coach or other team administrator from all rights and claims for damages, injury, loss to
person or property which may be sustained or occur during participation in Winskill Dolphins Swim
Club activities, whether or not damages or loss is due to negligence. I hereby acknowledge that my
children is (are) physically fit and capable of participation in all Swim Team activities.

2. Liability Waiver
By registering my child(ren) with the Winskill Dolphins Swim Club, I agree to participate (or allow my
child(ren) and family members to participate) in the Winskill Dolphins Swim Club, and hereby release
Winskill Dolphins Swim Club, its directors, officers, agents, coaches, and employees from liability for
any injury that might occur to myself (or to my child(ren) and family members) while participating in
the Winskill Dolphins Swim Club program, including travel to and from training sessions, swim meets
or other scheduled team activities.

I agree to indemnify and hold harmless the above mentioned organizations and/or individuals, their
agents and/or employees, against any and all liability for personal injury, including injuries resulting
in death to me, my child(ren) and/or other family members, or damage to my property, the property
to my child(ren) and/or other family members, or both, while I (or my child(ren) or family members)
participating in the Winskill Dolphins Swim Club program.

3. Swim BC Personal Information Consent
Personal Information Protection & Electronic Documents Act

CONSENT FORM - covering Swimming Canada, SwimBC, Winskill Dolphins Swim Club

A Parent or Legal Guardian must accept this agreement for those swimmers under the age of 18.
The Federal Personal Information Protection & Electronic Documents Act (and equivalent provincial
legislation) requires that consent be obtained prior to the collection and use of all personal information.
The personal information you provide to the Club from this registration will be used for the purposes
reasonably associated with the swimming activities conducted by the Club. These purposes include national,
provincial and event registration, insurance coverage, training and competition participation and competition
result publication. Some of the information you provide will be passed on to Swimming/Natation Canada (SNC)
and Swim BC, for purposes including association registration, insurance coverage and:

1. Ensuring swimmers train and compete in an age appropriate environment;
2. Establishing athlete eligibility for selection to swim teams;
3. Establishing pertinent medical records
4. Reporting non-identifying,
demographic and participation statistics to funders, sponsors and other
authorized third parties;
5. Publishing athletes? names, genders, ages, club affiliations on our web pages or in results, news
releases and ranking reports;
6. Making direct contact with swimmers as necessary for the operation of the Club, Swim BC and SNC.

Additional personal information may be collected from time to time. Consent for the use of this personal
information may be inferred where its uses are obvious and it has been voluntarily provided. When not
obvious, the purposes for collection will be provided prior to, or at the time of collection; either orally or in
writing.

for the Swim BC at www.swimbc.ca.

Should a swimmer wish to review their personal information held by the Club, Swim BC, or SNC they must
make a request to the appropriate organization pursuant to that organization?s Policy. Further, swimmers may
withdraw consent to use their personal information pursuant to the Policies. Such a withdrawal however, may
require the cancellation of your registration with and suspension of your activities with the Club, Swim BC and
SNC.

All swimmers or their legal guardian must sign a copy of this form each season.

I hereby consent to the collection and use of personal information as described above.

4. Acceptance of Club Policies
Winskill Dolphins Swim Club is a group of highly motivated, competitive, well-rounded swimmers. We are
dedicated to creating a community of like-minded individuals focused on responsible growth and development,
achieving excellence in and out of the pool.

Being a member of the Winskill Dolphins Swim Club is a privilege and it comes with responsibility. Winskill
Dolphins are establishing a reputation as a dynamic organization full of talent and enthusiasm. For us to reach
our full potential, all swimmers, parents and coaches must unite and work toward our common goals. When
our TEAM is successful, every individual within the team gains confidence and improves at a faster rate.

All Winskill Dolphins (Parents and Swimmers) should be familiar with the club’s Vision, Mission, Values,
Bylaws and Policies. By signing this waiver, every member is committed to becoming familiar with all club
expectations and policies and to becoming supportive, active and positive members of our community.

The WDSC Code of Conduct and Ethics aligns with Swim BC and Swimming Canada.

I CERTIFY, that I have read the CLUB HANDBOOK and that I and my family agree to abide by the policies of
the club as they are determined by the Board of Directors. I also accept that in matters of dispute that the
Board will have the final judgment.

5. Agreement of Payment
I certify that I understand and accept the following:

● A valid credit card MUST be registered to Member Account at all times. Cheques will not be accepted.
● My account will be kept in Good Standing - please see WDSC Bylaws
● I will fulfill the Parent Volunteer Commitments - please see Club Handbook
● I understand the Medical and/or Early Withdrawals policies - please see Club Handbook

I also permit the club to invoice my account and charge my credit card for any unfulfilled commitments as set
out in the Fee Summary.

6. Photo and Video Consent
I, being the Participant or the parent or legal guardian of the minor Participant, hereby grant to the
Organization the permission to photograph and/or record the Participant’s image and/or voice in pictures or
videos (collectively the “Images”), and to use the Images to promote the sport and/or the Organization
through traditional media such as newsletters, websites, television, film, radio, print and/or display form, and
through social media such as Instagram, Facebook, YouTube, and Twitter. I understand that I waive any claim
to remuneration for use of audio/visual materials used for these purposes. This consent will remain in effect
in perpetuity.

I, being the Participant or the parent or legal guardian of the Minor Participant, hereby fully release,
discharge, and agree to save harmless the Organization, from any and all claims, demands, actions, damages,
losses or costs that might arise out of the collection, use or disclosure of the Images or taking, publication,
distortion of the Images, negatives, and masters or any other likeness or representation of the Participant
that may occur or be produced in the taking of said Images or in any subsequent processing thereof, including
without limitation any claims for libel, passing off, misappropriation of personality, or invasion of privacy.

I, being the Participant or the parent or legal guardian of the Minor Participant, UNDERSTAND AND AGREE,
that I have read and understood the terms and conditions of this document. On behalf of myself, my heirs
and assigns, I agree that I am signing this document voluntarily and to abide by such terms and conditions
